Stem Cell Therapy: What You Need to Know

What Are Stem Cells?

Stem cells are cells with the ability to renew themselves and, under the right conditions, develop into more specialized cell types. They are central to normal growth, blood formation, and tissue maintenance. However, the phrase stem cell therapy is used broadly in advertising and can refer to products with very different sources, processing methods, evidence, and regulatory status.

A treatment should not be evaluated by the words “stem cells” alone. Patients need to know exactly what product is being proposed, where it comes from, how it is processed, what condition it is intended to treat, and whether that use has been reviewed or approved by the U.S. Food and Drug Administration.

Established Uses Versus Investigational Uses

Blood-forming stem cells, also called hematopoietic stem cells, have established medical uses in transplantation for certain cancers and disorders of the blood or immune system. In the United States, FDA-approved stem-cell products are derived from umbilical cord blood and are approved for disorders affecting blood production.

By contrast, the FDA states that regenerative medicine products marketed for orthopedic problems including osteoarthritis, tendon injuries, disc disease, and common back, hip, knee, neck, or shoulder pain have not been approved for those uses. Research continues, but participation in research and receiving a commercially marketed treatment are not the same thing.

A listing on ClinicalTrials.gov or a statement that a facility is “registered with FDA” does not by itself mean a treatment is FDA-approved or legally marketed.

Products You May Hear About

Clinics may use terms such as bone-marrow aspirate concentrate, adipose-derived cells, stromal vascular fraction, umbilical-cord products, Wharton’s jelly, amniotic products, or exosomes. These products are not interchangeable. They differ in cell content, donor source, processing, sterility controls, proposed mechanism, and regulatory requirements.

Some procedures collect and process a patient’s own tissue, while others use donor-derived material. Being “autologous,” or obtained from your own body, does not automatically establish that a product is safe, effective, exempt from regulation, or appropriate for a particular diagnosis.

How to Evaluate a Proposed Treatment

  • Confirm the diagnosis: ask what is causing your symptoms and how imaging, examination, or laboratory findings support that conclusion.
  • Identify the product: request its exact name, source, manufacturer or processing method, and whether it contains living cells.
  • Ask about regulatory status: determine whether the product and proposed use are FDA-approved, part of an FDA-authorized clinical trial, or neither.
  • Review condition-specific evidence: testimonials and before-and-after stories are not substitutes for controlled clinical research.
  • Compare alternatives: discuss physical therapy, medications, lifestyle changes, established injections, surgery, or continued observation.

Potential Risks and Uncertainties

Risks depend on the product and procedure but may include pain, bleeding, infection, immune or inflammatory reactions, contamination, unintended tissue growth, neurological complications, or failure to improve. The FDA has received reports of serious harm involving unapproved regenerative products, including infections, tumor formation, and blindness.

There may also be indirect harm when an unproven treatment delays care that has better evidence. Financial risk matters too: many investigational procedures are paid out of pocket, and a high price does not establish quality or effectiveness.

Understanding Clinical Trials

Legitimate clinical trials use a written protocol, defined eligibility criteria, informed consent, safety monitoring, and oversight. Ask for the trial identifier, sponsoring organization, principal investigator, institutional review board information, and details about who pays for treatment and follow-up care.

A study listing should describe the intervention and research question clearly. If a clinic uses the existence of an unrelated trial to validate its own product, or asks you to pay substantial fees while making guaranteed claims, consider seeking an independent second opinion.

Questions to Ask Before Proceeding

  • Is this exact product approved for my condition?
  • If it is investigational, is treatment being provided under an FDA-authorized trial?
  • What evidence supports the proposed dose and delivery method?
  • Who processes the product, and what sterility and quality testing is performed?
  • What adverse events have been reported, and who manages complications?
  • What outcome is realistic, and how will it be measured?

Taking the Next Step

If you are considering a product marketed as stem cell or regenerative therapy, start with a diagnosis-specific evaluation and ask for the proposed treatment in writing. Review the evidence, regulatory status, alternatives, total cost, and follow-up plan. For an investigational product, an independent opinion from a specialist who is not selling the procedure can provide valuable perspective.
